Get Answers for a User

Get the per-question Answers for the given USER_ID across all lessons.

Pagination is applied at the underlying ActiveLesson document level: each page covers pageSize active-lesson documents and may flatten to many more answer rows in the response. Use updatedSince for incremental sync.
