Get Answers for a User on a Lesson

Get the per-question Answers for the given USER_ID on the given LESSON_ID.

Each item is a single answer to a single question, flattened from the underlying ActiveLesson record. Includes the question title for visibility. Use includeArchived to also include answers from previous attempts and includeEntryQuiz to include entry-quiz answers in addition to the exit quiz.
